Q Is it practical for a solo orthopedic practice to offer the small, extremity type of bone density scanners? What is the potential revenue for such a system?
A Yes, it certainly is. Incorporating bone mineral density (BMD) testing into your clinical practice will provide you with an additional tool to identify those at risk for future fractures due to low bone density as well as generating new revenue.

Currently available are two types of bone density measurement devices. The peripheral systems that have been designed to measure an extremity, such as the wrist, and the central DXA devices that measure the spine and proximal femur.

Central DXA scanners are the standard of care in osteoporosis assessment, quickly and precisely measuring bone mineral density and assessing vertebrae-- the key indicators of osteoporosis. These units do require a larger capital investment, but the reimbursement, which is approximately $140/scan, offers a break-even point of 10-12 patients per month. Extremity units may initially be appealing with a smaller capital investment, but the reimbursement is significantly less at approximately $40/scan with no way to monitor or diagnosis the patient. This is according to The International Society for Clinical Densitometry's published guidelines that state peripheral devices should not be used for diagnosis or patient monitoring.
