Cost Effectiveness of BMD Testing on Men

Journal of the American Medical Association Publishes Study Looks at the Treatment of Osteoporosis in Older Men

BEDFORD, Mass. (August 7, 2007 - Osteoporotic fractures are common among elderly men. One in three hip fractures occurs in men, and they are associated with as much morbidity and higher mortality than in women. Vertebral fractures are also quite prevalent among older men, and are strongly associated with subsequent hip fractures, other clinical fractures, and probably as much morbidity in men as in women. Caucasian men at age 60 have a 29% chance of experiencing an osteoporotic fracture during their remaining lifetimes1.

The first peer reviewed study to asses the cost-effectiveness of widespread bone densitometry screening and subsequent drug therapy to prevent osteoporotic fracture among elderly Caucasian men was published this week in the Journal of the American Medical Association (JAMA)2. Dr. John Schousboe and his colleagues conclude that bone densitometry testing followed by bisphosphonate therapy for those with osteoporosis may be cost-effective for older men.

The Schousboe study has important public policy implications. Even though the Surgeon General has highlighted the fact that osteoporosis is a significant health problem in men, very little bone mineral density (BMD) testing is currently done in men. The International Society of Clinical Densitometry (ISCD) has published guidelines for BMD testing in men, but very few men receive this service. It is hoped that this study will increase physician's use of appropriate BMD testing in men and spur public policy towards universally mandated reimbursement for BMD testing in older men, similar to what exists now for women.

Another factor that may help increase the treatment of male osteoporosis is the fact that alendronate (including Merck’s Fosamax), the most popular bisphosphonate for the treatment of osteoporosis, will likely be available in generic form in 2008.
